Fire-Dex Handbook Acknowledgement and Receipt

I have received a copy of Fire-Dex, LLC (“Fire-Dex”) Associate handbook.

I understand that this handbook is for informational purposes only, and that it is a guide for Associates concerning their responsibilities to Fire-Dex and regarding pertinent Fire-Dex policies, associate expectations, benefits and other employment-related matters. I further understand that this handbook is not all-inclusive, and that Fire-Dex can revise or eliminate any of the matters set forth in the handbook or add new matter to this handbook, with or without notice.

I understand that it is my responsibility to read and follow the manual. If I have questions concerning information in the handbook, I will bring them to the attention of my Supervisor, Human Resources or an Officer of the Company.

I have entered into my employment relationship with Fire-Dex voluntarily and acknowledge that there is no specified length of employment. I understand and agree that nothing in the Associate Handbook creates, or is intended to create, a promise or representation of continued employment and that employment at Fire-Dex is employment at-will, which either I or Fire-Dex can terminate at any time and for any reason, with or without cause, so long as there is no violation of applicable federal, state or local law. I realize that no official, Supervisor or representative of Fire-Dex has the authority to enter into any agreement on behalf of Fire-Dex for any employment for a specified period, or for any other specific terms or conditions of employment, unless such action is taken in writing duly authorized by the CEO, President and/or Board of Fire-Dex. Furthermore, I acknowledge that this handbook is neither a contract of employment nor a legal document.

This handbook and the policies and procedures contained herein supersede any and all prior practices, oral or written representations, or statements regarding the terms and conditions of my employment with Fire-Dex. By distributing this handbook, the Company expressly revokes any and all previous policies and procedures that are inconsistent with those contained herein.

I agree not to disclose confidential information and proprietary Fire-Dex information, either during or after employment. I further agree to return all Company property in my possession or responsibility should I ever leave employment with Fire-Dex.

I have received the handbook, and I understand that it is my responsibility to read and comply with the policies contained in this handbook and any revisions made to it.
